Terms and Conditions for Seven Night Condo Stay

  1. To validate this offer you must complete the attached Activation Form and mail it within 21 days of the Issue Date shown on your Activation Form along with a $75.00 refundable deposit (certified check or money order only) and a self-addressed stamped envelope made payable to Customer Choice Rewards via certified mail, to:

    If you fail to include a self-addressed stamped envelope, your activation form will be returned to you.
  2. This offer expires 21 days after issue Date, and is valid for two adults, 21 years of age or older, with a major credit card, for a seven night condo stay in a studio room.
  3. Approximately 30 days after receipt and processing of your competed Activation Form and refundable deposit, a Reservation Request Form will be sent, on which you will submit your preferred departure dates and destinations. Two (2) departure dates are required, in order of preference. You must also select two (2) different destinations. Your first selected date must be at least 90 days in advance, while your second date must be at least 45 days after the first selected date. You will be instructed to mail in your completed Reservation Request Form, together with a non-refundable $99.00 per person (assuming a confirmed reservation is secured) reservation processing see for taxes and resort fees.
  4. Your Resevation Request Form and non-refundable reservation processing fee must be mailed back to Customer Choice Rewards and postmarked within 30 days of the Issue Date on your Reservation Request Form.
  5. Upon receipt and processing of your completed Reservation Request Form and non-refundable reservation processing fee, a Travel Reservationist will contact you via phone. During the call, your hotel accommodations will be confirmed and your contact information will be verified. All reservations are based on avaliability. After speaking with a Travel Reservationist, you will be mailed a confirmation letter confirming you itinerary.
  6. After returning from your stay you are entitled to submit a refund for your $75.00 refundable deposit. This request must be submitted within 30 days of your return. Please allow forty-five (45) days for processing.
  7. Both travelers are responsible for having correct documents required for travel.
  8. Cancellations of confirmed reservations are subject to forfeiture of all paid fees and deposits. Cancellation penalties may apply and certificate will be ineligible for future use. No changes are permitted to confirmed reservations.
  9. Year-round arrivals are available but are based on availability. This offer is not valid on major holidays or a full 7 days prior to and following major holidays.
  10. Transportation, government imposed taxes, transfers, meals, or incidental expenses are not included in this offer.
  11. This certificate has no cash value and cannot be sold or transferred. Void where prohibited by law and not valid for sponsors, distributors, employees, family members of sponsors, distributors, and employees. This offer cannot be used concurrently with any other travel offer. Accommodations and destinations are subject to change and availability.
  12. Do not depart for your vacation until you have received your confirmation letter.
  13. Customer Choice Rewards does not assume any liability for damages, expense or inconvenience, nor for loss, injury, death or damage to any persons or possessions or on any aircraft, or in transit to and from accommodations/aircraft, or for any cause whatsoever due to delays, cancellations acts of nature, mechanical breakdown, strikes, or acts of God. Customer Choice Rewards also assumes no responsibility for any verbal or written representation made in conjunction with this offer by any distributor and/or their agents other than those included in this offer. Customer Choice Rewards will make every effort to accommodate your travel date but reserves the right to offer alternative dates and locations when necessary.

NO PURCHASER SHOULD RELY UPON REPRESENTATIONS OTHER THAN THOSE INCLUDED IN THIS CONTRACT. YOU MAY CANCEL THIS CONTRACT WITHOUT ANY PENALTY OR OBLIGATION WITHIN 30 DAYS FROM THE DATE OF PURCHASE OR RECEIPT OF THE VACATION CERTIFICATE, WHICHEVER OCCURS LATER. YOU MAY ALSO CANCEL THIS CONTRACT IF ACCOMMODATIONS OR FACILITIES ARE NOT AVAILABLE PURSUANT TO A REQUEST FOR USE AS PROVIDED IN THE CONTRACT. IF YOU DECIDE TO CANCEL, YOU MUST NOTIFY THE SELLER IN WRITING OF YOUR INTENT TO CANCEL BY RETURNING THE CERTIFICATE AND SENDING NOTICE TO:
